To
view your assessment online, you will need to log into
your personal account via the homepage.
Once
you have logged in, click "View Assessments"
on your main login screen.
Select
the assessment you wish to view and click the button,
it will appear on the screen in printable version on
the official USSF form.
If
you have recieved an assessment, but cannot find it
online, please allow the assessor some time to get it
posted online. If you have not recieved your assessment
after several weeks, you may contact him/her to find
out the status of your feedback form.
