Request an Assessment

Are you a registered referee and wish to request an assessment from a certified assessor?

ALL REQUESTS MUST BE MADE FIVE (5) DAYS PRIOR TO MATCH DATE.

Did you know: in order for an assessment to "count" for upgrade and grade maintenance, you must request the assessment from the State Director of Assessment?

Fill out the form below and submit your request to the State Director of Assessment.  All areas of the form must be filled out unless noted specifically otherwise. 

Not sure which type of assessment you need?  Review the following brief summaries to ensure you request the correct type of assessment:

Upgrade Assessment:
You would like to try and move to the next higher grade.  A formal assessment must be conducted by a certified assessor of the proper level, on a competitive match.  Please review the US Soccer Referee Adminstrative Handbook, available at www.ussoccer.com for details on the specific level of match for each grade level.

Maintenance Assessment:
All referees grade 7 or higher must receive annual maintenance assessments to keep their grade level.  Please review the US Soccer Referee Adminstrative Handbook, available at www.ussoccer.com for details on the maintenance requirements of each grade level.

Developmental Assessment:
An "informal assessment" to give you feedback on your performance.  Developmental assessments do not count for maintenance or upgrade, and are solely for the aid of the development and improvement of the referee.
